Blue Cross and Blue Shield of TX
Medicare
NLPC will file all Medicare Claims. Medicare assignment accepted. You are responsible for deductible and coinsurance amounts due, at the time of service.
If you are enrolled with a Private Fee for Service (PFFS) Medicare Advantage plan, you must contact the plan to find out if we are in-network. Each enrollment period you must provide our office with current billing information. If a referral is required by your plan it is your responsibility to obtain the referral from your assigned primary care physician. (See Financial Policy/Contracted Insurance.)

Medicaid
NLPC is no longer accepting new patients with Medicaid as their primary insurance.
Established patients are required to obtain all necessary referrals/authorizations from their assigned primary care physician. A current Medicaid card is required each visit.

Blue Cross and Blue Shield of TX
NLPC is contracted with Blue Choice of TX only. Major plans include Health Select, Federal and UT Systems. It is very important to check with your insurance company directly to determine if we are currently in-network with your BCBS plan.
If a referral is required by your plan it is your responsibility to obtain the referral from your assigned primary care physician (See Financial Policy/ Contracted Insurance.)
Your co-payment is due at the time of service and is a requirement in order to be seen at NLPC. You are also responsible for deductible and coinsurance amounts. These amounts are due at the time of service.
It is your responsibility to provide all insurance information to NLPC. This includes but is not limited to a current copy of your insurance card and subscriber information.

In-Network Plans
It is very important to check with your insurance company directly to determine if we are currently in your network. Please see our current Common In-network List. This is not an all inclusive list and may change periodically.

Out of Network
We will file to out of network plans if the plan has out of network benefits. Patients must call their individual insurance plan to find out if they have out of network benefits available.
If a referral is required by your plan it is your responsibility to obtain the referral from your assigned primary care physician.

Worker's Compensation (WC)
NLPC does accept referred WC patients, however we will not be considered your "Treating Doctor." You must maintain a "Treating Doctor" that handles your WC case. You must be referred to this office by your "Treating Doctor." Complete WC information must be obtained prior to your first office visit. This includes but is not limited to your employer, date of injury, claim #, adjuster, WC carrier and all necessary contact phone numbers.
Once your claim is finally adjudicated you must provide updated billing information such as private health insurance. (See Financial Policy/Worker's Compensation.)

Private Pay
NLPC accepts patients that do not have insurance. For your convenience we accept all major credit cards, checks and cash. A cash pay discount is available.
If a payment plan is needed you must contact the Business Office.
