NEW PATIENTS
NLPC would like to welcome you as a new patient!

All new patients must be referred from their primary care physician or the physician currently treating their condition. We require demographic information (address, phone, insurance, etc.) along with medical records and diagnostic studies (written reports of all MRI, CT, and X-rays, actual films are not necessary) prior to making an appointment. This is to ensure that our new patients are being referred to a physician that can truly help them.

This information can be faxed to our office or the referring physician can call us.

For your convenience, our new patient forms may be printed and completed prior to your first appointment. Once you have filled out the forms, they may be mailed or faxed back to the office. All forms can be downloaded below.

Please read our Privacy Notice, print out and complete the Acknowledgement of Receipt of Privacy Notice. This form not only acknowledges that you have reviewed and accept our policy, but it also will determine who may receive access to your medical information or be present during your visit.

Next, please print, fill out and return the New Patient Health History and the Financial Policy.
